High FSH levels

Hi Paul, My husband has a v. low sperm count >50K on 2 tests and has shown a high FSH level.

What are our chances of finding any usable sperm for ICSI from anywhere?

Sorry if this is a bit of an odd question.

Hi,

This is not an odd question at all - it's a very natural question under your circumstances.

Firstly with his sperm count did you really mean >(ie more than) 50K, or < (is less than) 50K? Either way, clearly there are some but too few sperm, to do an accurate count. Where any of them motile, I assume probably not.

As his FSH is very high then his testes have been malfunctioning for a long time, possibly all of his life. There are obviously still some isolated islands of sperm production around the testes and it may be possible to find these during his biopsy. Sometimes it takes more than one attempt to find sperm and this repeated surgery can be traumatic, both physically and emotionally (and for both of you). Often only a few inactive sperm are retrieved, less than 20-30, but these can be used for ICSI, as you do not need normal or motile sperm, just live sperm (there is a difference between live and motile).

However he appears to have more sperm in his samples than might be recovered from his biopsies, and these could very well be suitable for ICSI, without the need for surgery. Why don't you speak to the laboratory staff, preferably the embryologists about this? They will be able to tell you if they would be able to recover his seminal sperm and whether these would work with ICSI.
